What it’s for

A feedback page collects structured responses — star ratings, multiple-choice questions, free-text comments — from a scan, then shows a thank-you screen. It’s built for post-visit or post-purchase surveys: table tents asking about a meal, receipt stickers asking about service, or event follow-ups asking attendees to rate the experience. Feedback templates are structurally different from other page-builder categories: instead of general-purpose content blocks, they’re built from a fixed four-block sequence — Logo Section, Start Page, Custom Form, and Success Page — which is what makes the form-and-thank-you flow work.
As with the other categories on this page, “Feedback QR code” is reached through the same Build a Page flow, filtered to feedback-tagged templates — not a separate creation route.

Find a feedback template and reach the builder

Open Create QR Code and choose Build a Page

From QR Codes → Create QR Code (/qr/create), click the Build a Page card.

Filter the gallery by Feedback

On Choose a template (/qr/new), click the Feedback tag. At verification time there were 3 templates: Restaurant Dine-in Feedback, Product Feedback, and Professional Event Feedback.

Preview and select a template

Clicking a card opens a preview panel with the template’s name, description, category tag, and a Use this template button. Product Feedback was used for verification — a generic SaaS-style feedback form asking “What do you think of [Product]?”.

Land in the page builder

Selecting it opens the builder at /qr/new/landing?template=fb-product-saas. The canvas shows a mobile preview of a “Thanks for using [Product]!” screen with a Give Feedback button, and the left-hand blocks panel lists Logo Section, Start Page, Custom Form, and Success Page.

Reaching this canvas confirms the flow — publishing isn’t required to verify it. Edit the Custom Form block to add or change survey questions, then use Publish from the top bar when ready.

Creating one via the API

To create a feedback-style QR code through the API rather than the UI, use category ID 26 (labeled Feedback) — see Create QR Code for the full request schema and Category List for the complete ID-to-label mapping.
